REACH and its partners envision an integrated model of care that aligns health and related resources to offer efficient, affordable, comprehensive healthcare to uninsured and underinsured persons living in the Greater Richmond Metropolitan Area with a focus on primary care; streamlined access to specialty, diagnostic and other services; documented outcomes and return on investment community-wide; and, the infrastructure and financing to deliver a broad scope of services that will improve health outcomes and the quality of life.
