The North District Community Health Clinic (NDCHC) is a free clinic, serving the un-insured and underinsured residents of North Richmond and the surrounding areas.  Our unique location affords the clinic high visibility and access.  NDCHC is located in the Fifth Street Baptist Church, accessible to local bus routes, has free street parking, and is easy to reach from major traffic routes (Interstate 64, Interstate 95, Chamberlayne Avenue, and Brooklyn Park Boulevard).

The clinic has hours in both the morning and the afternoon (9am - 2pm M,T,W; 11am - 4pm Th) by appointments and walk-ins. Created and supported by the Fifth Street Baptist Church, the clinic is open to people of all ethnic and religious backgrounds.

The clinic provides the following services:
  · Medical checkups, women's health, and well-baby care.
  · School physicals and children's immunizations.
  · Disease screening, treatment, and prevention for high blood pressure, diabetes, high cholesterol, cancer, weight management, etc.
  · Healthy lifestyle counseling, nutrition education, smoking cessation, alcohol and substance abuse screening, and community based health fairs.
  · Referrals to local hospitals and community services for more intensive treatment and rehabilition.




From Interstate 95:
 
  1. Interstate 64 East to Mechanicsville Turnpike ( Route 360) exit.
  2. Take second exit, Route 360 East.
  3. Stay in left lane, turn left at first stoplight, Magnolia Avenue.
  4. Follow Magnolia Avenue one block past next light (Fourth Avenue), turn left at Third Avenue.
  5. Go two streets on Third Avenue to Victor Street, turn left onto Victor.
  6. Look for sign "Fifth Street Baptist Church" Clinic.
From City of Richmond:
 
  1. Chamberlayne Avenue north to Brooklyn Park Boulevard, turn right onto Brooklyn Park Boulevard.
  2. After approximately 3 miles, you will reach a 5-way traffic light at the corner of Second Avenue and Meadowbridge Avenue.
  3. Turn right onto Second Avenue, travel two streets to Victor Street.
  4. Turn left onto Victor Street.
  5. Look for sign "Fifth Street Baptist Church" Clinic.
